Before you can register for a qualifier in NorCal, you must do 3 things to be eligible.      Check Your Team’s Status

1) pay the qualifier fee of $75  – paypal form now available

2) Team Details Form (1 for each team) – online Google Doc Form  – no login required  Trouble accessing the link and you are a teacher – likely your district is blocking GoogleDocs.    You can access via smartphone or any open ‘net connection.

3) Playing at Learning must have received physical copies of all Liability and Consent waivers per table below  (and each name listed in the team details form needs to submit a waiver and each waiver form needs to be listed on the team details form – ie there should be a “one to one” match.

Please add your team number(s) to the outside of the envelope and on the top of each of the consent forms.

 

PLEASE DO NOT SCAN/EMAIL OR FAX any of the forms. The Liability Consent Waiver form must be physically mailed or dropped off at The Play Space (when we are there) or at the address above.     

 

If you drop waivers off –  (and some people just want to do this for some reason or another)  note that the address above is our house.  There is a lidded box (it is clear ie see through) on the porch to leave envelopes.  No need to ring the doorbell as it just makes our dog crazy.   And please be considerate of our neighbors and neighborhood.

 

CHANGES?  If you end up with any changes to the “Roster” – team member or coach/mentor, fill out a new Team Details Form AND mail the additional waivers that are needed

 

MULTIPLE TEAMS?  If you are managing forms for multiple teams, please paperclip or staple each team separately but they can all be mailed in one envelope.

PLEASE CHECK:     

The most common problems are:

1) Please check that all minors have a parent/guardian signature.

2) All team members must have at least a valid birth month and birth year entered  – a lot of time the birth year incorrectly has 2012 –  please check

3) Each and every person – minor and adult – must have their own separate waiver form.

4) Each team must have at least 1 adult coach – if you are coaching/mentoring more than one team, you only need one waiver but please include all of your teams in one envelope so that I can track this

WHY DO WE DO THIS:   In order to make it easy for our tournament organizers, we collect these items ahead of time so that this is taken care of and there are no problems related to this when the team shows up at their qualifier.   All team members and all coaches and all mentors must submit a signed waiver – all use the same waiver form – the coaches/mentors are the “participant” on their waiver.  

 

We are _not_ collecting tournament volunteer waivers at this time.  Tournament volunteers will have their waiver collected at the tournament itself (and for each one that they volunteer for).

 

 

Note that we (Playing At Learning) do not use or collect any of the personal information on these forms. We do confirm that all team members are age eligible and that all the forms have signatures and all teams have at least one adult coach…  At the end of the season, we then ship the waivers off to FIRST where they are used for program-related research data.

 

Finally, when you physically mail the envelope to us – please do not require a signature for delivery. We are not typically available at the time that these get delivered for our home office and it adds about 3 to 5 days for us to receive it as we have to wait for it to get back to a central processing office and then figure out how to make get there to sign for the envelopes within restricted business hours. What a pain. We have not had a problem with getting these via regular first class postal mail, so we would recommend that you just send them that way.
