2011’s Information  – not valid for 2012 season.

 

Before you can register for a qualifier in NorCal, you must do 2 things to be eligible.

1) pay the qualifier fee of $75 and

2) Playing at Learning must have received physical copies of WAIVERS and cover sheet listed in table below

 

Deadline to complete both of these items is basically NOW for Phase 2 tournament registration!

PLEASE DO NOT SCAN/EMAIL OR FAX any of these forms. They must be physically mailed or dropped off.     Hopefully by next year the process will change through FIRST HQ (ala FRC’s process) but for this year, these need to be physically sent to us. [we had thought this would be true already….]

 

CHANGES?  If you end up with any changes to the “Roster” – team member or coach/mentor, just mail an updated cover sheet with the additional waivers.

 

MULTIPLE TEAMS?  If you are managing forms for multiple teams, please paperclip each team separately but they can all be mailed in one envelope.

 

PLEASE CHECK:  The most common problems are:

1) Please check that all minors have a parent/guardian signature.

2) All team members must have at least a valid birth month and birth year entered  – a lot of time the birth year says 2011 –  please check

3) Each and every person – minor and adult – must have their own separate waiver form.

4) Each team must have at least 1 adult coach – if you are coaching/mentoring more than one team, you only need one waiver but please include all of your teams in one envelope so that I can track this

 

 

WHY DO WE DO THIS HERE:   In order to make it easy for our tournament organizers, we collect both of these items ahead of time so that this is taken care of and there are no problems related to this when the team shows up at their qualifier. All team members and all coaches and all mentors must submit a signed waiver – all use the same waiver form – the coaches/mentors are the “participant” on their waiver. The waiver form is the same one for team members as it is for coaches and mentors –  the difference is just who’s name is put on the first line.     We are _not_ collecting tournament volunteer or general parent or sibling waivers.  Tournament volunteers will have their waiver collected at the tournament itself (and for each one that they volunteer for).

 

 

Important to note is that we must have the form for this 2011-2012 season as it is significantly different and completely reformatted.  English and Spanish versions are available.

 

Include Waiver Cover Sheet for each team: Since the new liability consent waiver form does not ask for team number and that is how we track these forms, we have created a “Waiver Cover Sheet” that must be included separately for each team when you mail the forms. It is available in both RTF and PDF formats.

 

Every few days, I will be updating a 2011 team list table to note which waivers have been received. I am human, so if you suspect an error, just send me an email.

 

Note that we (Playing At Learning) do not use or collect any of the personal information on these forms. We do confirm that all team members are age eligible and that all the forms have signatures and all teams have at least one adult coach…

 

The qualifier will receive the cover sheet and will confirm numbers of team members and coach/mentors listed. At the end of the season, we then ship the waivers off to FIRST where they are used for program-related research data.

 

Finally, when you physically mail the envelope to us – please do not require a signature for delivery. We are not typically available at the time that these get delivered for our home office and it adds about 3 to 5 days for us to receive it as we have to wait for it to get back to a central processing office and then figure out how to make get there to sign for the envelopes within restricted business hours. What a pain. We have not had a problem with getting these via regular first class postal mail, so we would recommend that you just send them that way.
